ANNUAL MEMBERSHIP FEE SCHEDULE:
SINGLE CHILD • $1200 Child (0-24 months) ($100 month)
• $1000 Child (25 months and Up) (less than $85 month)
SECOND CHILD • $600 (only $50 month)
ADDITIONAL CHILDREN • $300 (only $25 month)

NOTE: Personalized Pediatrics is not an insurance company. Fees cover only the personalized, special services provided by this practice.

If not covered by a participating insurance company, patients will incur a fee per visit in addition to the annual membership fee.
