ANNUAL MEMBERSHIP FEE SCHEDULE:
SINGLE CHILD • $1400 Child (0-24 months) ($116.66 month)
• $1200 Child (25 months and Up) ($100 month)
SECOND CHILD • $800 (only $66.66 month)
ADDITIONAL CHILDREN • $400 (only $33.33 month)
OFFICE FEES:
SICK VISIT • $150.00 (Monday to Thursday regular office hours)
• $175.00 (Weekend, after hours and holidays)
WELL VISIT WITH VACCINES • $200
WELL VISIT WITHOUT VACCINES • $175

NOTE: Personalized Pediatrics LLC is not an insurance company. The yearly fees cover the personalized services. They are not reimbursed by any insurance company. The individual visit fees are separate and patients will incur these fees per visit. If the patient has insurance the individual visit fees may be reimbursed. Ask for details.
